2002 – Diagnosed with CHF (Congestive Heart Failure)
2007 – Pacemaker/Internal Cardiac Defibrillator (ICD) implanted
2013 – Pacemaker/ICD replaced – battery depleted
November 2014 – Right Atrial Cardiac Ablation preformed
February 2016 – Mid Circumflex Stent implanted
April 2016 – LVAD implanted and 2 heart valves repaired and aortic valve replaced
August 2016 – Cataract Surgery – Left eye
November 2016 – Cataract Surgery – Right Eye
January 2017 – Bacterial Blood Infection
March 2017 – Compression Fracture of L3 vertebra
April 2017 – Kyphoplasty of L3 vertebra
April 2017 – Compression fracture of L2 vertebra
May 2017 – Kyphoplasty of L2 vertebra
May 2017 – Pacemaker/ICD replaced – battery depleted
June 2018 – RAMP Study
September 2018 – Oral Milrinone Study
